Student Records
The Office of Disability Resources is the university office charged with collecting and maintaining student disability documentation. This information is kept in a secure file with limited access and is only shared with others with the written permission of the student. Information will only be shared on a limited basis within the university community if there is a compelling reason, such as a threat to an individual’s safety and/or emergency situation.
Maintenance of Records
A confidential file is maintained on each student that includes documentation of the disability, and a record of each contact and action taken. Student records are shredded five years after the last contact.
Request for Copies of Educational Records
Students are advised to keep personal copies of their documentation. Students may request a copy of their documentation by completing the Educational Records Request form. The ODR will provide copies of documentation generated by this office, but is not able to forward documentation that originated with another institution or professional.
To make a formal request, please fill out and return the Copy of Disability Educational Records Request Form.
